Full Immunizations Required

The student is 4 years old and may proceed with enrollment and placement.
  • 4 doses - Polio (IPV)
  • 5 doses - Diphtheria, Tetanus, Pertussis (DTap)
  • 2 doses - Measles, Mumps, Rubella (MMR)
  • 3 doses - Hepatitis B (Hep B)
  • 2 doses - Varicella (Chickenpox)
Born 2/12/2022 - 7/22/2022

Pending Immunization

The parent must present an updated IZ record before July 22, 2026; the student will not be admitted without it.
  • 4 doses - Polio (IPV)

Conditional Immunizations

The student may be admitted with the IZ for 3 years of age, and follow-up will be required.

  • 1 doses - Measles, Mumps, Rubella (MMR)
  • 3 doses - Hepatitis B (Hep B)
