Original U.S. High
School Diploma, GED (or foreign equivalent), Military DD214 or AA degree
or higher with official transcripts.
3.
Original Social Security
Card AND Driver's License or ID, Resident
Card, Work Visa (Resident
Card and Work Visa needed only if it
applies to your Citizenship status).
4.
Original Tuberculosis
clearance from a U.S. doctor or clinic. We also accept any providers under
SIMSA.
5.
Current Adult and Child
CPR/AED/First Aid Card. Must be obtained through the American Heart
Association or American Red Cross only and cannot be an online course.
