Student Insurance and Consent Collection Form
Your health insurance is the key to ongoing student wellness support.
The Chula Vista Elementary School District (CVESD) is dedicated to supporting the health and wellness of every student. CVESD supports this by having a School Social Worker/Counselor at each site who offers services such as counseling, social-emotional learning support, and other behavioral health resources that promote their overall well-being.
Families are asked to complete the Student Insurance and Consent Collection Form to support these efforts. The information collected allows CVESD to access state funding through the Children and Youth Behavioral Health Initiative (CYBHI), a California program that strengthens school-based health services.
Complete the Student Insurance and Consent Form here, available in both English and Spanish.
