How to Register

STEP 1: Complete the online eligibility form.

In order to begin the enrollment process, you must complete the online eligibility form. Click on the Eligibility Form link below to complete this step.

STEP 2: Send and attach documents listed below.

To determine your eligibility, be sure to send all documents listed below by email, scan, or photograph as an attachment to: [email protected]

Your registration will not be processed until all documents are received.

  • Child's birth certificate and birth records of all your children under 18 in the household (for family size verification).
  • Child's immunization record (child you are enrolling only).
  • Parent(s) Identification (Driver's license, Government issued ID, Passport).
  • Proof of residence in the parent's name dated within 30 days. Mobile cell phone and/or past due bills are not accepted. It can be one of the following: mortgage statement, lease agreement, rent receipt, apartment complex letter on letterhead signed by the landlord, homeowner's association, correspondence from government agency, gas/electricity, water, trash, sewer, cable and telephone bills (land line only).
  • Pay stubs for the last 30 consecutive days, cash aid, food stamps, section 8, pension, unemployment, and/or child support or any other source of income. If none of these apply to you, please see below:

Self-Employment Declaration Form: If you receive income from a job or business and you do not have an employer who pays payroll taxes for you, you are self-employed. Some common self-employment examples are; childcare provider, contractor, uber/lyft driver and independent hair stylist. Click on the Self-Employment Declaration Form link to complete this form and we will need your most recent tax return.

Employer Cash Form: If your employer pays you cash or personal check or if you have customers that pays you with cash, please click on the Employee Verification Form link to complete this form and attach your most recent income tax return (if applicable).

Self-Declaration for Parents/Guardians with Non-Working Income: Use this form if you do not have a job outside of home or if you are living on savings. Use section A if you are non-working spouse (and are not receiving EDD) and you live with family who support you financially. Use section B if you are living out on savings. Click on the Self Non-Working link to complete this form.

STEP 3: Complete the packet after receiving a call from preschool office.

If you were notified by preschool office that your family qualifies for the program, please complete the registration packet. Either email it to [email protected] or drop it off in our mailbox located in the CVESD Preschool Office at 84 E J Street, Chula Vista 91910 (our office is located behind the main building).

What happens next?

After completing step 1 and 2, an enrollment specialist from the preschool office will contact you within the next two weeks
by (email or phone call) to finalize your child's pre-enrollment. Please note, in order to expedite the registration process, all
documents listed in step 2 must be emailed to our department. Be advised that you will need to provide us with verification
of residency and income for the month of May 2023.

Priority placement is given to students deemed at-risk/recipients of child protective services and then to student who will enter kindergarten the following school year. Applications will be processed, and placements will be assigned based on rank assignments and space availability. Approved families will be contacted to complete full registration.
