Please read about the required documentation for new kinder registration. Registration will begin on March 12 at 7:30 a.m. in the school auditorium. Eligibility for the 2008-2009 school year is for children who will be 5 on or before December 2nd, 2008.

Dear Parents,
Welcome to Olympic View! Kindergarten registration is an exciting time for both parents and children. We know your time is valuable and we do need you to realize that the registration process does take some time, so please be prepared to wait. We want to make sure that all of the paperwork is in order, and we want to be able to answer any questions you may have. Only parents or legal guardians can register a child for school. If you are living with another family and do not have any proof of residency in your name, you must bring the homeowner/renter with you to register. Please call the school office, 656-2030, for specific details.
If you do not have all of the required documents, or if all of the required paperwork is not filled out, or your child does not have all of the required immunizations, you will not be able to register your child.
In order to register your child on or after March 12, 2008, you will need to have the following documents with you:
□ Your child’s birth certificate or passport
□ Your child’s immunization record with the following required immunizations correctly documented:
• Polio – 4 doses
• DPT – 5 doses
• MMR – 2 doses
• Hepatitis B – 3 doses
• Varicella – 1 dose OR verification from the health provider that the child has had chickenpox
□ Your proof of residency-any two of the following with your name and address, dated within the past 60 days, and not Past Due:
• utility bill (gas & electric, water, sewer, and/or trash)
• mortgage papers or escrow papers
• HOA statement
• rental contract and current rent receipt (this is considered only one item)
• verification of Social Services (either SSI, AFDC, or Medi Cal only)
□ Parent or guardian picture ID
□ Papers showing guardianship if you are the child’s legal guardian.
The following forms in your packet need to be filled out:
Registration forms (several forms stapled together) – all forms
Verification of Residency (pink) – entire form. The school year is ’07-08’
California School Immunization Record (blue) – top section only
The letter regarding Child Health and Disability Prevention – sign and date
